There is a pin, think of it like a cotter but one solid piece that holds the door handle on.

Push back on the door card behind the white plastic grommet and pull the pin out with a pair of needle nose pliers. That will free up the handle to slip off.

I think you'll need to remove the top piece on the door that the window pegs slip into.

Then just take off the screws around the rest of the door card and that should do it.

I just use a drift pin and a small ball peen hammer to re seat the pin holding the door handle on.

I believe that mounting it on the forward part of the door is going to prevent you from putting it on any way but at an angle. In order to get the base of the mirror straight or level is to mount it forward of the door on the body with the mirror even with or just forward or behind the windshield. Here's a photo of mine and even on the body it isn't perfectly straight:
