I used to have one. I mailed it to Indiana (first picture). I got it from a shop in Oregon, damaged a little from age, and it wasn't a good fit for the CMC body.
Jim (my build partner) and I made this (second and third pictures) out of aluminum and a pair of Thunder Ranch fiberglass 550 headrests. It wasn't hard, and the pictures are in my file. Easily enough done.
